There have been so many questions about what should be in a 504 plan that I thought I would share with you exactly what is in our plan. These are just mere suggestions on what has worked well for us. We have used this plan from Kindergarten up through High School and have had to make just minor tweaks! Hope you find it helpful. Feel free to share this vlog with your diabetes communities.
